Prescreening Questions
  1. Are you 18 years or older?
  2. Are you having a single baby during this pregnancy (not twins, triplets, etc.)?
  3. In the past five years, can you agree that you have not engaged in high-risk behaviors such as illegal drug use or sex in exchange for money or drugs?


Can you answer “yes” to the questions above? If so you may qualify to donate your baby's umbilical cord blood with HCBB.

If you have had any of the following within the past year, a tattoo, ear or skin piercing, non sterile acupuncture, accidental needle stick, and/or a blood transfusion, please contact us at (808) 983-2265, for further screening.


How Do I Become A Cord Blood Donor?

Review the prescreening questions listed above. If you answered "yes" to the above questions, and you are at least 6 months pregnant, you may choose one of the following options:

Option 1: Request that an enrollment packet be mailed to you by calling the Hawai'i Cord Blood Bank office at (808) 983-2265.

Option 2: From this website, you may download all the forms (located on the right) and mail the completed forms to:

Option 3: When you check into the Labor & Delivery Unit at the hospital to have your baby, you may ask your nurse for an enrollment packet.

PLEASE NOTE:

If your cord blood unit is collected & sent, 6 vials of blood (~3 tbsp.) will be drawn from you for testing after delivery (
please see consent form).

Checklist, Short Screening Form, and Consent Form


** Approximately 32% of all our collected units meet the criteria for banking and transplantation.

If your unit is selected for banking, a full screening medical and social history form needs to be completed within 7 days of your delivery, as these units will be going to patients with very serious illnesses.

Without this time-sensitive full screening form, your unit will be
discarded.

Usually we complete this form with you
after delivery, however, if you wish to fill out the form beforehand please contact our office at (808) 983-2265.

Please note that after submission, we may still have to contact you if there are any questions that need clarification.
